Step 1:
Go to http://www.whatismybrowser.com.
This report notes your Operating System version, web browser & version, IP address, screen & browser size, and Silverlight version.
Step 2:
Email this information to the requested address.
Scroll down to the bottom of the page and enter the appropriate fields.
